The following information is required to be submitted by the specified student groups to provide effective delivery of care and to ensure a safe campus environment for all. Students must using their Monarch account username (King's College email) and password for detailed instructions.
Resident students must submit:
Student health history
Physical exam
Immunization history with Tuberculosis (TB) Screening Questionnaire*
Insurance information
Commuter and off-campus students must submit:

Medical history and health insurance information (strongly recommended)
Meningitis Requirements
The Commonwealth of Pennsylvania requires that students living in college-owned and operated residence halls must either provide proof of meningitis immunization or sign a waiver that they understand the risks and choose not to be immunized. King’s College mandates this vaccination at or after the age of 16.
Students who fail to submit proof of meningitis vaccination or a signed waiver will not receive a room key or be able to move into their residence hall.
Health insurance coverage is mandatory for full-time resident students, athletes, and international students and strongly recommended for all other students.
Proof of health insurance (a clear image of the front and back of your health insurance card) should be uploaded on the Student Health Portal.
Students Without Insurance
King's College refers domestic students and athletes to various agents for insurance options and purchases that are individually structured for students who have no other form of health insurance or need additional local or sport insurance coverage. Please be sure to fully disclose your needs to the agent when investigating purchase options.
King's College provides accident insurance for all students. This insurance coverage is an excess policy. A claim will be filed for an accident occurring on campus; however, a student's health insurance is always primary.
In the event of injury, you should contact or report to the Student Health Center as soon as possible to complete the Accident Claim Form.
*The TB Screening Questionnaire determines who is at high risk and needs to submit a (TB) test prior to arrival on campus.
